Why Am I Always Tired? A Naturopath’s Guide to Persistent Fatigue

You’ve tried sleeping more. You’ve cut back on caffeine. You’ve even dragged yourself to the GP, only to be told your blood tests are “normal.” Yet here you are – still exhausted, still struggling to get through the day.

If this sounds familiar, you’re not alone. Persistent fatigue that doesn’t respond to basic lifestyle changes is one of the most common concerns I see in my Perth Naturopath clinic. The good news? There’s usually a reason behind it, and finding that reason is exactly what naturopaths are trained to do – treating the root cause of fatigue.

Fatigue is a symptom, not a diagnosis. When someone comes to me with long-standing, resistant tiredness, I don’t just look at one thing, I look for patterns. These patterns tell a story about what’s happening inside the body, and they point me toward the right tests and treatments. Two of the most common patterns I see are low iron and thyroid dysfunction. Both can leave you feeling completely drained, but they present differently when you know what to look for.

Low iron doesn’t just make you tired, it creates a specific cluster of symptoms that, together, paint a clear picture. Clients with low iron often describe:

  • Low exercise tolerance
  • Feeling wiped out after minimal effort
  • Heart palpitations, especially when standing or exertion
  • Hair loss, particularly noticeable on your brush or in the shower
  • Feeling heavy and sluggish, like your body is weighed down
  • Struggling to wake up in the morning, no matter how long you’ve slept

When I see these symptoms clustered together, it gives me a strong starting point. Low iron is one of the first things I want to investigate, and it often gets missed because standard blood tests might show your levels as “within range” even when they’re not optimal for you.

Thyroid dysfunction creates a different pattern entirely. If your fatigue comes alongside any of these signs, your thyroid may be worth looking into:

  • Feeling cold all the time, even when everyone around you is comfortable
  • Gaining weight easily or finding it almost impossible to lose weight, despite doing all the right things
  • Constipation that doesn’t resolve with dietary changes
  • Puffiness, particularly in the face and around the eyes
  • Losing the outer third of your eyebrows, which is a classic clinical sign that many people don’t know about

This combination of symptoms is what I’m trained to recognise, and it’s often the missing piece for clients who’ve been told everything looks fine on paper.

When a client comes to me with fatigue that just won’t shift, I don’t guess. I use a process of elimination.

That starts with asking a lot of questions. Not just “how tired are you?” but detailed questions about your sleep quality, your digestion, how your body responds to exercise, whether you’re losing hair or feeling cold, what your periods are like, and how long this has been going on. Sometimes the circumstances surrounding the initial onset of your fatigue are a major clue.

These questions help me identify patterns, and those patterns point me toward the right kind of testing. I then order targeted pathology to back up my clinical thinking, so we can be confident we’re addressing the actual cause rather than wasting your time or money on treatments that don’t match what’s really going on.

This is something I’m passionate about. When I was a lecturer at Endeavour College of Natural Medicine, a significant part of my role was teaching students how to recognise exactly these kinds of patterns in their clients, so they could achieve the best treatment outcomes.
